Cedar Lake is a private not-for-profit organization that originally incorporated out of common concern for persons with intellectual and developmental disabilities, and the lack of adequate residential facilities within our geographic area. Cedar Lake was founded in 1970 by Louisville-area parents of persons with intellectual disabilities. Cedar Lake Lodge began providing campus services in 1974, and added “community-based” service options, Cedar Lake Residences, in 1989. Job Skills/Requirements

Purpose/General Description
To provide each resident with continuous, professional healthcare services by administering treatments, medication, etc., as prescribed by the physician. To document doctors orders, treatments and care given in an organized, systematical manner. Initiate and follow preventative measurements to protect residents from undue injury, illness or trauma with the goal of developing, maintaining, and improving the residents physical well-being which will enab! le them to Purpose/General Description

To be a member of the team of the nursing staff/therapists and all CLL staff by effectively communicating verbally and in writing. To knowledgeable of and apply current state and federal rules and regulations.

ESSENTIAL FUNCTIONS
1. To participate in the resident's individual program plan and ensure the health services aspects of that plan.
2. To assess, plan, implement, and evaluate the resident's treatment pla! n.
3. To promote wellness and provide prevention techniques.
4.! To provide holistic health care which includes emotional, spiritual, physical and safety aspects.
5. To observe, record, and report findings to the appropriate person.
6. To perform nursing procedures according to doctor's orders with accepted professional practices.
7. To assist with rehabilitation by working with various departments and therapists.
8. To attend various resident related meetings as needed/directed.
9. To give and receive verbal report to the next shift, to read the flowsheet report from the previous shift.
10. To make rounds on each shift to observe and confer with various staff members on the resident's treatment plan.
11. To perform routine nursing duties including but not limited to giving medications and treatments, taking vital signs, monitoring blood sugars, and urine testing.
12. To call the physician as sign and symptoms occur. Requires medical follow-up or
Hospital visit.
13. To administer basic first aid a! nd/or CPR in an emergency situation.
14. To report to the Director of Health and Habilitation Services as the situation warrants.
15. To document all drugs and treatments given on the appropriate chart per facility procedures.
16. To assist the staff physicians in visits to CLL and to accompany residents to the doctor or hospital, as needed.
17. To investigate promptly any symptom brought to the nurse's attention by habilitation staff or family.
18. To answer parents' inquiries regarding their resident's situation in a professional, confidential manner.
19. To write shift notes on the flowsheet for the next shift.
20. To write monthly nursing summary, and assessment in the resident's chart.
21. To communicate with various staff and therapists as the situation warrants.
22. To assist the Director of Health and Habilitation Services in any way possible for the welfare of the residents.
23. To order medication and work closely with the p! harmacist.
24. To be the shift charge person as the schedule warran! ts.
25. To perform other duties as assigned.
