The Referral and Admission Coordinator is responsible for responding to new referrals to identify needs, offer hospice solutions, and admit medically eligible patients into Hosparus car! e. The Referral and Admission Coordinator coordinates care and services, educates referral sources, patients, and their families about the hospice benefit and options available. The Referral and Admission Coordinator strives for the goal of providing premier customer service.
ESSENTIAL DUTIES
ROLE SPECIFIC CRITERIA
Assessment

  • Meets with patient/family to identify needs and offer hospice solutions.
  • Assesses the patient to ensure medical eligibility for hospice care.
  • Reviews relevant medical records, assessing for documentation to support hospice medical eligibility.
  • Evaluates coverage/relatedness of care/treatment/services based on the hospice patient care and treatment coverage guidelines. Obtains pre-authorization for all care/treatment/services needed at admission.
  • Understands and is able to explain to patients/families various payer sources and the requirements for coverage by those payer sources.
  • Begi! ns the interdisciplinary assessment based on immediate patient! /family needs.
  • Identifies immediate education needs of the patient/family and addresses those needs.
Planning
  • As appropriate during admission, may begin the process of preparing family/caregiver for the dying process, including signs and symptoms of impending death and notification of Hospice at the time of death.
  • Completes requests for non-required services (CNA, Volunteer, etc.) & initiates plan of care, when service need is identified upon admission.
  • Completes the admission Plan of Care, in coordination with the patient/family.
  • Proactive approach to patient/family needs.
Evaluation
  • Evaluates patient/family outcomes and satisfaction with care/services provided.
  • Identifies possible improvements in department/hospice admitting functions in order to increase access and promote efficiency.
Professional Efficiency
  • Completes all admission paperwork with patient/f! amily, targeting the goal of admitting patients using the appropriate visit design model.
  • Documents using point of service in a concise manner all pertinent data that reflects referral/admission contact.
  • Voicemails appropriate teams & or departments only pertinent data.
  • Collaborates with the Team Director to assure completion of duties, scheduling and arranging coverage for department.
Collaboration
  • Builds collaborative and collegial relationships with Referral and Admission Team members, Interdisciplinary Teams, Preauthorization team, the Hospice Finance Department, and others to assure coordination and continuity of care and services.
  • Mediates and solves problems effectively. Communicates with all community partners and referral sources, as well as with families in a positive manner, consistently seeking win-win solutions.
Education/Teaching
  • Begins the education process regarding all aspec! ts of care to be delivered to the patient and/or primary caregiver.
  • Serves as a professional mentor/role model.
  • Assumes the role of preceptor when requested.
Required Experience:Experience:
  • 5 years of nursing experience; ER, ICU, Home Health experience a plus.
  • Knowledge of Medicare/Medicaid reimbursement helpful.
  • Knowledge of palliative care helpful.
Required Education:Education and training and licensing requirements:
  • RN with current RN licensure in the states of Kentucky and Indiana (or able to obtain licensure in the state of Indiana within 60 days of hire).
