Carries out activities ! related to utilization management, discharge planning, care coordination and referral to other levels of care. Works with physician, social worker, and interdisciplinary team to facilitate clinical pathways and achievement of desired outcomes. Promotes interdisciplinary collaboration and champions service excellence. Works collaboratively to ensure patient needs are met and care delivery is coordinated across the continuum at the appropriate level of care. Indirect, remote, periodic supervision. Establishes own daily work schedule, sequencing work in order of importance and using independent judgment to meet the needs of the patients.

Required Skills

Licensure to practice as a Registered Nurse in the Commonwealth of Kentucky. Bachelors degree in Nursing preferred. Knowledge: Clinical expertise, Excellent interpersonal communication and negotiation skills, Strong analytical, data management and computer skills, Current working knowledge of discharge plannin! g, utilization management, case management, performance improv! ement and managed care reimbursement, Able to work with people of all social, economic, and cultural backgrounds, Flexible, open-minded and adaptable to change, Understanding of pre-acute and post-acute venues of care and community resources, Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components, Ability to work independently and exercise sound judgment in interactions with physicians, payors, and patients/families.

Required Experience

Must have worked in an acute care hospital for at least five years. Two years case management experience preferred.
