Job Details:
POSITION SUMMARY

The Director of Care Coordination is a clinician who is knowledgeable in all aspects of care management and the transitions of care between ambulatory, acute, and post acute care. The Director will have responsibilities not only in the acute care setting, but also in other areas in the continuum of care. The Director will work to ensure integration and cooperation of services including, but not limited to, Case Management and Social Work Services. The Director understands the clinical documentation needs of payers and organizations in the continuum of care to meet the patient's needs during and post discharge from the acute care. The Director is familiar with regulatory requirements related to care management and clinical documentation and facilitates compliance.

Care coordinat! ion ensures a patient's needs and preferences for care are und! erstood, and that those needs and preferences are shared between providers, patients, and families as a patient moves from one healthcare setting to another. The effects of poorly coordinated care are particularly evident for people with chronic conditions such as diabetes and hypertension and those at high risk for multiple illnesses who often are expected to navigate a complex healthcare system.

The Director position requires a strong commitment to service excellence, positive clinical outcomes, cost efficiency, performance enhancement, and high business acumen.

Job Requirements

1. Licensed as a Registered Nurse required.
2. Bachelor's Degree minimum. Masters degree preferred..
3. Minimum of 5 years management experience required.
4. Certification in Case Management preferred.
5. Strong organizational and time management skills, as evidenced by a capacity to prioritize multiple
tasks and role components.
6. Ability to col! laborate with and understand the needs of C-level executives and translate those needs
into actionable results and plans.
7. Ability to work independently and exercise clinical judgment in interactions with physicians, payers,
the patient and their families.
8. Excellent interpersonal communication and negotiation skills.
9. Strong analytical, data management and computer skills.
