Join! our team at the Masonic Home of Louisville’s Sam Swope Care! Center. We are seeking both RN's /LPN's with experience working in long term care and short term rehabiliation to be accountable for delivering exceptional care to our residents.
This brand new 196,000-square-foot center replaced the existing 83-year-old nursing home, adding an array of services in a community environment unlike any other in Kentucky. We are seeking both RN's /LPN's with experience working in long term care and short term rehabiliation to be accountable for delivering exceptional care to our residents.
This brand new 196,000-square-foot center replaced the existing 83-year-old nursing home, adding an array of services in a community environment unlike any other in Kentucky. The Care Center’s unique design features six households connected to an interior Main Street, featuring outdoor sky imagery ! by National Geographic. With a library, arts and crafts studio, café, gift shop, salon and chapel, Main Street is a destination for residents, families and staff. Outdoor features include waterfall courtyard and playground.

Job Skills/Requirements
Performs nursing functions within the long term or short term households that requires registered nurse / licensed practical nurse with experience in managing care for senior residents.
This fast paced environment includes duties such as providing excellent care and service. Communicate issues to the Nurse Leader, or the physician as needed and follow up.
The household nurse works closely with the Nurse Leader or Clinical Mentor to ensure regulatory compliance a Performs nursing ! functions within the long term or short term households that requires r! egistered nurse / licensed practical nurse with experience in managing care for senior residents.
This fast paced environment includes duties such as providing excellent care and service. Communicate issues to the Nurse Leader, or the physician as needed and follow up.
The household nurse works closely with the Nurse Leader or Clinical Mentor to ensure regulatory compliance and a person-centered and resident-directed culture. Ensures the clinical operations in the rehab household are in compliance with federal, state and local regulation, the Nurse practice Act of the state and professional standards of nursing care. Participates as household clinical staff in hiring functions, orientation, training, clinical competency, coaching, education, counseling and performance appraisals, maintained in a manner supportive of team and professional standards of practice.
Minimum of 3 years experience required.
Education Requirements
- Licensed Practical Nurse (LPN) by State of Kentucky
- Licensed Registered Nurse (RN) by State of Kentucky
- Active CPR certification
