Overview

To evaluate, implement and/or facilitate services that address the needs of clients and/or caregivers with the objective to assist clients to remain independent and community-based to the extent possible. As guardian, the case manager would manage the client’s medical services, living arrangements and finances under the general direction of the Program Director.

Job Skills/Requirements

1. Bachelor’s Degree in social work, gerontology or related field.
2. One year demonstrated experience in social services to adults.
3. Qualified life experience may be substituted for direct practice.
4. Excellent skills in verbal and written communication.
5. Ability to enter client data in computer systems and effectively utilize Microsoft Office.
6. Valid driver’s license and proof of insurance coverage.

RESPONSIBILITIES:
1. Assist cli! ents to access or coordinate, needed services or benefits, including referrals to other agencies, such as, but not limited to food stamps, medical assistance and Medicaid, housing, seasonal programs and mental health services.

2. Implement care plans making modification as necessary while coordinating referrals and on-going services.

3. Maintain regular contact with clients through office and/or home visits and telephone contact wi RESPONSIBILITIES:

1. Assist clients to access or coordinate, needed services or benefits, including referrals to other agencies, such as, but not limited to food stamps, medical assistance and Medicaid, housing, seasonal programs and mental health services.

2. Implement care plans making modification as necessary while coordinating referrals and on-going services.

3. Maintain regular contact with clients through office and/or home visits and telephone contact with the client and/or the caregiver.
4. Provide “fee-for-service” Elder Life Navigator services to families caring for aging relatives in compliance with program goals and objectives.

5. Conduct on-going re-assessment of the needs of care recipients and their families, as well as elder care consultations.

6. Provide informal education to clients or their families in budgeting and self-help skills and knowledge of appropriate resources as appropriate.

7. Participate in patient care conferences for clients in long term care facilities and in multi-disciplinary team conferences to ensure clients’ care needs are met.

8. Consult with treating doctor’s, nurses, specialists, hospital staff and other professionals to evaluate appropriateness of treatment, placement, and level of care.

9. Authorize all medical treatment, including DNR, with required documentation when appropriate.

10. Make “end of life” arrangements when appropriate or needed.
11. Provide financial services to clients by monitoring income and e! xpenses, budgeting needs of clients, including payment of bills, in conjunction with agency program outcomes.

12. Engage client’s family when possible and maintain contact through letters, e-mail, telephone contacts, and office conferences; establish visitation schedules when indicated and in the best interest of the client.

13. Assist in Court or legal activity including preparation of affidavits, court testimony, communication with officers of the court, adult protective services, and/or law enforcement.

14. Assist in researching client assets, including, but not limited to, insurance, funeral, CD’s, bonds, vehicles, personal or real property and other information as required for the Inventory, Plan to Preserve and Annual Reports due to Disability or Probate court.

15. Prepare a plan for preservation or liquidation of the client’s estate to ensure funds are available for continued care of the client.

16. Maintain confiden! tial case records in a timely and organized manner.

17. Perform other duties as assigned.
