POSITION SUMMARY :
Under t! he direction of the Supervisor of the Senior Advocacy Customer! Center, the individual is responsible for contacting patients in disease-specific programs to perform telehealth calls and quality of life surveys. As well as making satisfaction calls to discharged patients or their responsible party to obtain feedback on services received from the agency while utilizing the highest professional standards.

POSITION RESPONSIBILITIES:
1. Contacts patients active in disease-specific programs to perform telehealth calls and quality of life surveys to assist in measuring patient outcomes.
2. Performs follow up calls to patients that have been discharged or their responsible party using a standardized script and procedure to obtain feedback on services received from the agency.
3. Demonstrates a strong customer service focus, handling calls with the highest professional standards and compassion.
4. Communicates with appropriate office with feedback from customers to improve services, provide recognition of staff ! and identify continued positive health status.
5. Works collaboratively with patients, caregivers, peers and leadership.
6. Perform other customer service related activities/calls as assigned.

1. Prior experience in a call center role is required; experience in healthcare setting is preferred.
2. Excellent phone skills and etiquette are required.
3. Proficient in Microsoft Office (Word, Excel, Outlook); prior experience with proprietary computer programs/databases preferred.
4. Proven ability to manage workload, remain flexible and complete tasks accurately and timely.
5. Demonstrated ability to multi task and manage different customer focused projects as needed.
6. Excellent verbal and written communication skills with keen ability to convey information to internal and external customers in a clear and concise manner.
7. Strong attention to detail and active listening skills.
8. Ability to cope with ambiguity and frustration whi! le remaining 100% engaged with the patient.
