This professional leads and manages a hospital-based team of professional staff, composed primarily of nurses and social workers. Under the leadership of the director care management this team is responsible for providing utilization review services, care coordination, discharge planning and psycho-socials services to the hospitalâs patient population. The director care management leads the team in continuous process ! improvement activities to ensure the hospitalâs patients rec! eive cost-effective, high quality and safe care.
The director of clinical effectiveness and care management is responsible for tracking, trending and reporting to senior leadership department metrics that reflect the hospitalâs financial performance and patient satisfaction.
As a matrix position, the director care management collaborates with both hospital leadership and with Norton Healthcare system leadership to create and achieve strategic goals and initiatives.
As a member of the system's leadership team, the clinical effectiveness and care management director serves as a role model for the Norton Healthcare Service Basics that support service, communication, teamwork and safety.
Registered Nurse, Masters Degree in Nursing or Masters Degree in related field is required. Active Nursing license in the Commonwealth of KY is required. Certified Case Manager (CCM) or Accredited Case Mgr (ACM) is desired and current CPR certification is r! equired. Five years clinical experience in medical, surgical or critical care Nursing and three years management experience is required. Three years experience in utilization review or case management is desired. One to two years of process improvement initiatives as a leader/facilitator or participant including identification and monitoring of key performance and process measures relate to those initiatives is desired.
