This job reports to the Household Coordinator .
Relocation is not provided and travel is not required.
This is a Full-Time position , working 1st Shift .
Overview
Join our team at the Masonic Home of Louisville�s Sam Swope Care Center. We are seeking a LPN with a minimum of one (1) year or more experience working in long term care to be accountable for delivering exceptional care to our residents.
This brand new 196,000-square-foot center replaced the existing 83-year-old nursing home, adding an array of services in a community environment unlike any other in Kentucky. Job Skills/Requirements
Performs nursing functions within the rehab household that require a licensed practical registered nurse with experience in managing care for short term rehab residents.
This fast paced environment includes duties such as providing excellent care and service. Communicate issues to the Nurse Leader, or the physician as needed and follow up.
The household nurse works closely with the Nurse Leader or Clinical Mentor to ensure regulatory compliance and a person-centered and resident-directed Performs nursing functions within the rehab household that require a licensed practical registered nurse with experience in managing care for short term rehab residents.

The household nurse works closely with the Nurse Leader or Clinical Mentor to ensure regulatory compliance and a person-centered and resident-directed culture. Ensures the clinical operations in the rehab household are in compliance with federal, state and local regulation, the Nurse practice Act of the state and professional standards of nursing care. Participates as household clinical staff in hiring functions, orientation, training, clinical competency, coaching, education, counseling and performance appraisals, maintained in a manner supportive of team and professional standards of practice.
Education Requirements
- Nursing Degree
- Licensed Practical Nurse (LPN) by State of Kentucky
- Active CPR certification
