Overview
Join our team at the Masonic Home of Louisville's Sam Swope Care Center. We are seeking an RN/ LPN with experience working in short term care to be accountable for delivering exceptional care to our residents.
This brand new 196,000-square-foot center replaced the existing 83-year-old nursing home, adding an array of services in a community environment unlike any other in Kentucky. We are seeking an RN/ LPN with experience working in short term care to be accountable for de! livering exceptional care to our residents.

Job Skills/Requirements
Performs nursing functions within the short term care household that requires registered nurse / licensed practical nurse with experience in managing care for senior residents.
This fast paced environmen! t includes duties such as providing excellent care and service. Communicate issues to the Nurse Leader, or the physician as needed and follow up.
The household nurse works closely with the Nurse Leader or Clinical Mentor to ensure regulatory compliance and a pers Performs nursing functions within the short term care household that requires registered nurse / licensed practical nurse with experience in managing care for senior residents.

The household nurse works closely with the Nurse Leader or Clinical Mentor to ensure regulatory compliance and a person-centered and resident-directed culture. Ensures the clinical operations in the rehab household are in compliance with federal, state and local regulation, the Nurse practice Act of the state and professional standards of nursing! care. Participates as household clinical staff in hiring functions, or! ientation, training, clinical competency, coaching, education, counseling and performance appraisals, maintained in a manner supportive of team and professional standards of practice.
Basic qualifications:
Minimum of 2 years working in short term rehab
Education Requirements
- Nursing Degree
- Licensed Practical Nurse (LPN) by State of Kentucky
- Licensed Registered Nurse (RN) by State of Kentucky
- Active CPR certification
