The Brook Hospital is currently seeking a Evaluation and
Referral Coordinator Second Shift 3p-11p to join our team

The purpose of this position is to answer all incoming intake
calls and provide appropriate resolution. Conducts initial
evaluations to determine an appropriate treatment level of care.
Responsible for completing admission paperwork, including admission
data entry. This position requires a person to work with minimal
supervision. This position will work with all hospital units,
business office, medical staff, medical records, utilization review,
marketing, nursing, and data processing.

Requirements

The following are job requ! irements

for the position Evaluation and Referral Coordinator:
Flexibility is needed but the
business needs for this position are hours of 3p-11p Monday through
Friday

Answers telephone within 3 rings
using the appropriate greeting and attempts to obtain all pertinent
clinical, demographic, and insurance information from the caller to
determine caller needs. Shows awareness and manages calls waiting in
the que promptly

Uses therapeutic and crisis
intervention skills to obtain resolution for the caller

Explains the evaluation and
admission process to the potential patient/referral source and
informs patient of necessary items to bring for the hospital
stay

Demonstrates professional behavior
and interpersonal skills reflective of the facility mission statement
and philosophy

Computer skills are required for
data entry into several systems

This position! requires a Master's
degree in the behavioral science field, advanc! ed certification is
preferred such as LCSW, LMFT, LPCC
