RN Case Manager

Location: London Kentucky

PHHCA, a leading provider of home health care services, is currently seeking qualified applicants for the positon of RN Case Manager. This position reviews medical records to assure quality control measures and standards of care are met. Assists in providing on-going educational programs on documentation, coverage requirements and other staff development p! rograms. Participates in the medical reimbursement process to ensure timely and accurate filing of claims.

QUALIFICATIONS:

1. Holds current state license in the State of Kentucky and/or the State of Tennessee as a Registered Nurse or Licensed Practical Nurse.

2. Demonstrates working knowledge of regulations and reimbursement for home health care.

3. At least one year home health nursing experience, preferred.

4. Case Management, utilization review and supervisory experience preferred.

5. Ability to travel extensively during all seasons.

6. Position requires the physical ability to perform the job functions outlined under the “Duties and Responsibilities” listed below.

7. Must have a current drivers license, the ability and willingness to travel, and reliable transportation.
