Purpose: Delivers professional nursing care for clients in accordance with the established plan of care and agency policies and procedures. Provides care consistent with all relevant professional standards and state and federal requirements.Essential Functions: 1. Assesses, plans and intervenes using the nursing process to meet the client's health care needs.2. Observes and reports any signs or symptoms indicative of changes in condition of the client and family situation and documents such reports and observations in the clinical record.3.
Performs assigned nursing proced! ures, including the administration of treatments and medicatio! ns in the care of the client, in accordance with the State Nurse Practice Act and the plan of care.4. Interacts with clients and families to meet individual physical, psychosocial, and spiritual needs.5. Teaches the clients/caregivers appropriate self-care techniques.6. Assists with the rehabilitation of clients according to the client care plan.7.
Consults the Case Manager/MD in making changes or decisions with the plan of care.8. Maintains compliance with all the requirements of state licensure and Medicare Conditions of Participation.9. Notifies the Case Manager, physicians, and families of changes in the client's condition.10. Maintains data collection, recording, and reporting systems to ensure proper service, uniform accounting, data collection, and measurement of outcomes.11.
Participates in on-call and committee meetings on request.12. Communicates potential risk management issues and reports abuse or unsafe/illegal practice as required by law.Posi! tion Requirements: Education: Graduate of an accredited registered nursing programExperience: Home care experience as required by state law.Minimum of 1 years experience in nursing preferred, with a combination of acute care and home health care/public health preferable.Special Skills: Knowledge, ability and the necessary skill and judgment to perform all nursing procedures as outlined in the scope of practice.Ability to contribute as a team member and work independently.Excellent verbal and written communication skills, strong interpersonal skills.Knowledge of the regulatory requirements at the state, federal, and local level.Flexible with diverse client assignments.Licensure: Current license (in good standing) as a Registered Nurse in the state(s) of practice.Current CPR certificationCurrent driver's license from the state of residence
