Description: Hosparus is in need of Community Nurse Practitioner right now, this position will be placed in Kentucky. The Community nurse practitioner (CNP) is assigned to a specific Hosparus community location and reports to that communityâs medical director. The NP provides direct patient care, including accepting patients as the hospice attending practitioner, making medically-necessary visits (billable) and face-to-face recertification (FTF) visits (non-billable) and rotating to cover general Hosparus-wide weekend call for orders and prescriptions. In addition, the NP supports and coordinates care with the Interdisciplinary Group (IDG) and participates in education, outreach and leadership activities! for the community location. The CNP serves as a support and b! ack-up to the Community Manager. The CNP is also actively engaged in community outreach, building awareness and acceptance of the Hosparus brand. The CNP assists the CL in providing overall office leadership and management. Assures cost effective and clinically effective care and assists with developing, implementing and evaluating current and new programs and services. Manages community relationships and collaborates with outreach staff in performing marketing and outreach initiatives. Maintains knowledge and compliance with organizational policies, state and federal regulations, and accreditation standards pertaining to hospice care.
ESSENTIAL DUTIES
ROLE SPECIFIC CRITERIA
LEADERSHIP: Responsible for representing the organization in a professional manner and providing staff with a positive role model.

4. Responsible for being familiar with Hosparus Policies and Procedures and practicing according to these standards and guidelines. When unsure, accesses the Hub (share point) for online review of the policy and procedure detail, and consults with supervisor or subject matter experts as needed.
5. Works collaboratively with the community manager to achieve Hosparus community and organizational goals, and ensure delivery of quality services to patients and families.
6. Works collaboratively and collectively with Hosparus support departments, i.e. Customer Support, Preauthorization, Referrals, etc. to ensure effective and efficient customer focused responses.
7. Assures compliance with all licensing, ! certification, accreditation and regulatory standards as they pertain t! o the clinical areas.
COMMUNICATION: Demonstrates effective written and oral communications that engage internal and external audiences, and assures staff is kept well informed of organizational issues and plans.

QUALITY PATIENT CARE: Responsible for ensuring patients and families receive high quality of care.
1. Provides consulting and attending patient care services, including medically-necessary visits with appropriate documentation, coding and billing capture when designated as hospice attending practitioner, medical orders, prescriptions, family meetings, and coordination of care with hospice IDG staff and Hosparus or community physi! cians.
2. Participates in Hosparus-wide weekend general on-call rotation for urgent orders, prescriptions and telephone consultations.
3. Maintains adequate medical records and completes them in a timely manner.
4. Serves as a clinical resource to admissions and team members and ensures quality outcome for patients and families from the time of referral through the familyâs bereavement time.
5. Ensures compliance with organizational policies and procedures and Federal/State regulations for case management, required visits, coordinated interdisciplinary group work and meetings, and plan of care development and updates.
6. During IDG meetings, facilitates discussion to assess patientâs concurrent eligibility for hospice services, and work with the physician, team, pt/family to complete discharge planning when indicated
7. Convenes Complex Case Reviews in a timely manner, on patients where there are concerns that are not able to be resolved in an ID! G meeting.
8. Ensures a timely response to calls for assistance, qu! estions or need for support from patients, families, referral sources, community partners, and facilities.
9. Responds immediately and openly to patients/families/caregivers that have a complaint or concern about the care they have received.
10. Effectively utilizes and engages the Hospice Medical Director/Physician and community attending practitioners to ensure excellent quality of care for patients.
QUALITY & COMPLIANCE: Responsible for ensuring quality and compliance.
1. Develops and maintains an environment that promotes continuous quality improvement, including an atmosphere where problems are discussed openly without personal accusations, brutal facts are acknowledged and data is used to measure outcomes.
2. Working with the management team promptly investigates and responds appropriately to internal and external customer complaints, and ensures issue is logged in the electronic tracking system.
3. Responsible for having a thorough unde! rstanding of the regulations that affect Hosparus (COPS, state regulations, HIPAA, etc.) and practicing in accordance with these regulations.
4. Supports the community manager in evaluating the communityâs quality outcomes and implementing improvement initiatives.
CONTINUOUS LEARNING & TEACHING: Responsible for professionally growing employees and self.
1. Pursues growth and development through educational opportunities, professional networking associations.
2. Contributes to and learns from state and national hospice endeavors through participation in IHPCO, KAHPC, the NHPCO or other professional organizations as appropriate.
SUPERVISION & DEVELOPMENT OF STAFF & VOLUNTEERS: Through education and mentorship supports the development of staff and volunteers.
1. Empowers staff by providing needed information, encouraging and facilitating team participation in decision-making; and supports employees to demonstrate self-accountability in e! stablishing their work practices and making judgments within the organi! zationâs practice and policy expectations.
2. Displays regular appreciation for staff/volunteers and their contributions to the department and the organization by using thank you notes, Over the Top & Gold Leaf awards appropriately.
ADMINISTRATIVE DUTIES: Works in a conscientious, consistent and thorough manner. Demonstrates concern for thoroughness and accuracy. Collaborates with other peers and leaders.
1. Performs and documents Medicare-required face-to-face re-certification visits in full compliance with Medicare regulations.
2. Provides education to physicians, allied health care providers and general public in the community.
3. Participates in education, performance improvement, outreach and leadership activities, at the discretion of the medical director.
PROFESSIONAL EFFICIENCY:
1. Demonstrates clinical expertise in advanced nursing practice.
2. Maintains current state-of-the-art knowledge and skills in hospice an! d palliative advanced nursing practice.
3. Works toward or maintains certification by the National Board for Certification of Hospice and Palliative Nurses.
4. Maintains appropriate state licenses and controlled substance permits.
5. Maintains required continuing education units and advanced practice credentials.
6. Complies with state laws for prescription of controlled and non-controlled medications.
7. Maintains provider status with third party payers, as applicable
8. Participates in policy setting by attending quarterly meetings of the Medical Advisory Committee.
9. Performs and documents re-certification face-to-face visits and attestations in a timely manner.
10. Presents at and participates in Journal Club on a regular basis.
COLLABORATION:
1. Establishes a collaborative practice agreement with the medical director regarding prescriptive authority
2. Collaborates with the medical director or associate medical d! irector for prescription of controlled substances and management of com! plex cases that are outside the scope of advanced nursing practice.
3. Participates in building relationships with community physicians and nurse practitioners
4. Participates in Hospice committees and work groups, at discretion of the medical director.
Required Experience:
Experience
- Minimum of 1-year experience in full time advance nursing practice required.
- Primary care, palliative care, hospital-based, or hospice experience preferred.
- MSN or higher level Adult Nurse Practitioner required.
- Licensure (or eligibility for licensure) for advanced nursing practice in both KY and IN, including DEA and controlled substance permits, required.
