The Inpatient Unit Registered Nurse is responsible for direction, supervision, coordination and evaluation of all aspects of hospice nursing care provided in the Hosparus Inpatient Care Center within basic policies of the Clinical Services Department ! and overall department goals. Demonstrates excellence in nursing practice and oversees the hospice care provided by CNAs and Unit Secretaries.
ESSENTIAL DUTIES ROLE SPECIFIC CRITERIA Assessment
- Conducts a thorough nursing assessment upon admission to the Inpatient Unit.
- Assesses the need for all care, treatment, and services.
- Identifies educational needs of the patient and family.
- Utilizes knowledge of Hosparus Patient Care Guidelines and Clinical Protocols in the development of nursing interventions.
- Communicates and collaborates with the patientâs attending physician regarding care and treatment.
- Involves patient and family in the patientâs Plan of Care.
- Demonstrates clinical competency in principles of symptom control, pain control, and palliative care.
- Implements all nursing care and treatments identified in the Inpatient Unit Plan of Care.
- Assumes responsibility for all patient transfers from t! he Inpatient Unit to other settings.
- Prepares patients and caregivers adequately for discharge from the Inpatient Unit.
- Prepares family and caregiver for the dying process, including signs and symptoms of impending death.
- Provides follow up after the death and closure as in consultation with Inpatient Unit Social Worker and Chaplain.
- Evaluates patient response to care and treatment.
- Participates in the evaluation of department functions, identifying trends and areas for improvement as appropriate.
- Documents in a concise manner all pertinent data that reflects patient and family care; this includes all medications, treatments, and point of service documentation in the electronic documentation system.
- Administers all medications according to Hosparus Louisville Medication Administration Standards.
- Implements Admission Standing Orders according to po! licy.
- Assures that the Medication Profile is updated and correct prior to discharge.
- Collaborates with the Director of Patient Care to assure completion of duties.
- Work reflects productive and efficient practice according to departmental goals and expectations.
- Count narcotics with a nurse from oncoming shift and resolve any PYXIS discrepancies prior to the end of the shift. Involve assistance from Clinical Manager/Charge Nurse as needed.
- Communicates clearly, concisely and in a timely manner with team members.
- Collaborates with team members, including Interdisciplinary Team staff, Bereavement staff etc., to assure coordination in delivery of care.
- Demonstrates collaborative and collegial relationships with the Interdisciplinary Team.
- Works with Inpatient team members to assure cost-effective care.
- Provides education regarding all aspects o! f care to be delivered to the patient and family.
- Serves as a ! professional mentor.
- Assumes the role of nurse preceptor as needed.
- One year of nursing experience required.
- Knowledge of Hospice and or palliative care preferred.
- RN with current RN licensure in the state of Kentucky.
- BSN preferred.
- CHPN encouraged.
